Form Designer and
Miraplacid Form Viewer. Scan a form, load it to
Miraplacid Form Designer,
add input fields and push a button to open the form in
the Miraplacid Form
Viewer. Fill the form and print it.

You can save form layout and fill the form again later.

Miraplacid Form is free supported software.
Microsoft .NET required.
